Authorizations

• All services not listed on the Preferred IPA Direct Referral form require prior authorization. 

• Contracted PCPs and Specialists may directly refer patients for Direct Referral services to Contracted Providers without first obtaining prior authorization from the Utilization Management Department

• The current Direct Referral form is available on the Preferred IPA website.

• To get the Direct Referral form, navigate to:

  • Provider Services

  • Then Authorizations

  • Then Direct Referral form

• Or by accessing this link: https://www.preferredipa.com/provider-services/authorizations

• Contracted PCPs and Specialists may also submit Direct Referrals through the Provider Portal.
